TB-500 and thymosin beta-4: what separates the research?

TB-500 research is a good example of how easily two closely related substances get mixed up. One has been tested clinically. The other has not.

What has been seen in studies in humans?
Here it is necessary to separate two substances. The clinical studies concern thymosin beta-4 as eye drops, not TB-500 as an injectable research preparation.
In a phase 3 trial for neurotrophic keratopathy, an uncommon corneal disease, healing of corneal damage was measured in patients who received drops with thymosin beta-4 or placebo. The trial reported improved healing compared with placebo. An earlier phase 2 trial examined dry eye.
We have not found any controlled study in humans in which TB-500 was used for tendons, muscles or other tissue outside the eye. The evidence base for that area consists of animal studies, laboratory experiments and mapping reviews.

Thymosin beta-4 and TB-500 are not the same thing
Thymosin beta-4, often written Tβ4, is a protein the body naturally produces, with 43 amino acids. It binds actin, a structural protein inside cells, and has been studied for its role in tissue repair.
TB-500 is a synthetic substance that corresponds to an active part of thymosin beta-4. It is therefore a fragment, not the whole molecule.
That distinction matters when reading studies. Almost all clinical research concerns thymosin beta-4, not the fragment.
What has been tested clinically?
A randomized phase 2 trial published in 2015 in Clinical Ophthalmology examined eye drops with thymosin beta-4 for dry eye, in a model with controlled environmental exposure.
A placebo-controlled phase 3 trial published in 2022 in International Journal of Molecular Sciences examined eye drops with the same substance for neurotrophic keratopathy, an uncommon corneal disease, and reported improved healing compared with placebo.
A study on pressure ulcers has also been registered in clinical trial registries, see registry entry NCT00382174. A registered study without published results is not evidence of effect.
What do the eye studies mean for other uses?
The results concern drops on the cornea for specific eye diseases. That is a local formulation on a very well-defined tissue.
The fact that a substance heals the cornea in drop form does not mean the same substance affects tendons or muscles in some other way. Formulation, tissue and disease state differ.
For musculoskeletal use there are mainly animal studies and review articles, such as a scoping review published in 2026 in Applied Sciences. A scoping review maps out what has been written, and it does not replace clinical data.
Thymosin beta-4 also appears as an add-on in the small retrospective knee study from 2021, where four of 16 patients received it together with BPC-157. There it is not possible to determine what was potentially due to which substance, and the study lacks a control group.
Proposed mechanism
Thymosin beta-4 binds actin and affects cell mobility, and is also described as being able to promote the formation of new blood vessels and reduce inflammation in damaged tissue.

Limitations
The biggest limitation is that evidence for one substance is often presented as evidence for the other.
