BPC-157: extensive animal research and small human studies

BPC-157 research is often mentioned in connection with the healing of tendons, muscles and the intestinal lining. Most of the evidence comes from animal experiments, but there are also a couple of small published studies in humans.

What has been seen in studies in humans?
There are two small published studies in humans. One is a retrospective review of knee pain from 2021, in which 17 patients were identified and 16 could be reached for follow-up. Of these, 14 reported that their pain had eased.
The other is a safety pilot study from 2025 with two participants, who received infusions on day one and day two with blood tests and monitoring also on day three. No measurable effects were seen on markers for the heart, liver, kidneys, thyroid or blood sugar, and no side effects were reported.
The evidence base is therefore limited. Neither study had a control group or random assignment, the number of participants was small and the follow-up was short. This is not enough to establish either effect or long-term safety. The animal studies, which are far more numerous, are described in their own sections further below.

What is BPC-157?
BPC-157 is a synthetic peptide with fifteen amino acids. The sequence originates from a protein found in gastric juice, which is where the name body protection compound comes from.
The substance is therefore not identical to any hormone the body naturally produces. It is a fragment assembled in a laboratory.
What have the animal studies examined?
Most of the research has been done in rats and mice, often in models where an injury was deliberately induced. The studies have concerned wound healing, tendon injuries, muscle injuries, ligament injuries and injuries to the stomach and gut.
A systematic review published in 2025 in HSS Journal went through its use in orthopedic sports medicine. The conclusion was that preclinical studies point to possible benefit for musculoskeletal injuries, while controlled clinical trials are lacking.
A systematic review is a review of existing studies following a predetermined method. It does not create new data, but it shows what the evidence base looks like as a whole.
What do the studies in humans show?
There are small published studies in humans. One is a retrospective review of knee pain published in 2021 in Alternative Therapies in Health and Medicine. The researchers reviewed records from a private clinic in Florida and identified 17 patients who had received BPC-157 injected into the knee joint. Of these, 16 could be reached for follow-up by phone, so the results are based on 16 responses out of 17 included patients.
Of the 16 who responded, 12 had received BPC-157 alone, and 11 of them reported clear improvement in pain. Four patients had received BPC-157 together with thymosin beta-4, and three of them reported improvement. In total, 14 of 16 reported relief.
Here is how the numbers should be read: patients were asked afterwards by phone, there was no control group and no random assignment, the follow-up time varied, and no measurement instruments were used for function or quality of life. A design like this cannot distinguish a treatment effect from natural progression, placebo effect or recall error.
The other is a safety pilot study published in 2025 in the same journal. Two adult participants received BPC-157 as an infusion on day one and day two, with monitoring of vital signs and blood tests around the infusions. On day three, further testing and monitoring were done. No measurable effects were seen on markers for the heart, liver, kidneys, thyroid or blood sugar, and no side effects were reported.
Two participants in a short study is a very small evidence base. The fact that no side effects were noted there says nothing about safety with broader use, other doses, other routes of administration, or longer time periods. Rare side effects only become visible in much larger data sets.
Why are larger trials still missing?
A review published in 2026 in Pharmaceutics describes the practical obstacles to developing BPC-157 as a drug, including questions about stability, formulation and the regulatory path forward.
In other words: it is not just a matter of willingness. A substance needs documented manufacturing quality, pharmacological data and funding for clinical trials before its effect in humans can be properly tested.
How is BPC-157 thought to work?
The mechanisms described in animal studies include the formation of new blood vessels in damaged tissue and effects on growth factors that govern healing.
These are mechanisms observed in animal models. They explain how an effect could arise, but they do not show that it does arise in humans.
Limitations and risks
It is common for a substance to look promising in rodent models. Many such substances then show no effect in controlled trials.
- The published human studies are small and lack a control group, which means a treatment effect cannot be established.
- Long-term safety in humans has not been studied.
- The studies use specific formulations and routes of administration, for example injection into the knee joint or intravenous infusion, which cannot readily be compared with each other.
- Animal studies often use injury models that have no direct equivalent in humans.
